MEDICAL SERVICES

PHYSICAL THERAPY​

 

​Our physical therapy team develops an individual treatment plan based on clinical need. Our clinicians work with patients to improve and restore their functional mobility, increase muscle strength, increase gait stability, improve transfer skills, assist in pain management, and facilitates in acquiring proper durable medical equipment.

OCCUPATIONAL THERAPY

 

​Our occupational therapists guide and instruct the patient in prescribing therapeutic self-care and creative activities that are directed toward improving independence, physical, and cognitive functioning. Specifically, our occupational therapists can help with fall prevention, ensuring the patient's home is safer with environmental modification recommendations, and implementing appropriate adaptive/assistive devices and/or splints to promote optimal level of functioning.

SPEECH THERAPY

 

Our speech therapy team identifies, educates and treats: functional speech defects, oral motor skills, memory strategies, cognitive impairments, and swallowing difficulty. Our therapists' goals are improving client function with a patient specific plan.

SKILLED NURSING

​

We have professional nurses that are kind, compassionate, and patient focused. They go through an extensive orientation and training process before seeing patients and are provided the most up to date educational tools for their scope of care.

MEDICAL SOCIAL SERVICES

 

We have medical social workers that provide services and/or establishes standards addressing the social and/or emotional factors that impact the patient’s ability to achieve the goals of their plan of care. Our team educates clients/families about community resources and how to access them, prepares social histories to augment existing service, or as a guide in determining or changing level of service.  This may include cultural factors, financial concerns, and support system.

HOME HEALTH AIDE

 

Our home health aide is assigned to specific clients by the case manager and performs services for clients as necessary to maintain their personal comfort. Services include but are not limited to: bathing, skin/nail care, oral hygiene, shaving, dressing, and light housekeeping.

THERAPY & REHABILITATION

We work in conjunction with the patient's physician and family to ensure there is an individualized care plan to meet the patient's therapy and rehabilitation goals. Our total team approach is one-on-one in-home therapy to improve range of motion, progressive mobility, muscle strengthening, and continuous education and communication.

ORTHOPEDIC CARE

Our team educates clients/families about community resources and how to access them, prepares social histories to augment existing service, or as a guide in determining or changing level of service.  This may include cultural factors, financial concerns, and support system.

WOUND CARE

 

We offer wound care for all phases of acute, critical, and chronic wounds, including but not limited to: pressure ulcers, venous leg ulcers, diabetic foot ulcers, and post-operative wounds. We use the most up to date and state of the art wound care supplies resulting in the most optimal healing environment.

POST HOSPITALIZATION CARE​

 

Our goal is to keep our patients in the comfort of their home. Our case managers strive to make the transition from the hospital to home seamless.

PAIN MANAGEMENT

 

Pain is assessed and evaluated on every home care visit. Our team works closely with the patient's physician to develop a comprehensive pain management regimen. We ensure patients have the tools and resources to achieve optimal comfort.

CARDIAC TEACHING & ASSISTANCE

​

Our services for conditions include but are not limited to: congestive heart failure, hypertension, coronary artery disease, myocardial infarction, and atrial fibrillation. Our team assists in disease management by teaching the patient and family on the disease process, medication teaching/management, diet recommendations, PT/INR monitoring, and weight monitoring.

DIABETIC TEACHING & TRAINING

 

Diabetes can be a difficult chronic disease for patients to manage, but controlling blood sugar levels in order to prevent serious complications is imperative. With our program we help with glucometer use, diabetic foot care, medication management, dietary guidelines, a disease management education.
